Transfer Certificate & Fund Contacts
The industry agreed transfer certificate must be completed by the health fund of a member that is transferring to another health fund within 14 days of the notice period given by the member. If the member does not nominate a receiving health fund, then the transfer certificate must be issued to the leaving member to present to their new health fund.
